 Orthodontic Clear Aligners in Medically Complex Patients: A Breif Case Report on Renal Dialysis





Managing orthodontic patients with renal failure on hemodialysis requires a careful balance of clinical precision and patient safety. These cases demand modifications at every level—from material selection to treatment mechanics.
I recently managed a case using Maxflex™ trilayer clear aligners, and I'd like to share the key protocols that guided a safe approach.
The Medical Problems:
Altered bone metabolism
Increased bleeding risk (due to heparin during dialysis)
Higher susceptibility to plaque accumulation and infection
Potential toxicity concerns from aligner sheet materials
📋 My Modified Protocol:
Timing: Procedures were scheduled on non-dialysis days to minimize bleeding risk. While this was not always fully, due to the patient's fluctuating health status, I adapted accordingly.
Plaque Control & Attachments: Based on clinical research and expert opinion, attachments were entirely eliminated from the treatment plan. This decision was made to reduce plaque retention sites, as these patients are highly susceptible to plaque accumulation and gingival inflammation.
Aligner Trimming: Though scalloped trimming is typically recommended, I opted for a straight trimline placed 1 mm above the gingival margin. This modification was necessary to maintain adequate aligner retention in the absence of attachments.
IPR (Interproximal Reduction): IPR was completely avoided to eliminate the need for antibiotic prophylaxis or additional medications, reducing pharmacological burden on the patient.
Toxicity & Material Safety: I used Maxflex™ trilayer premium-quality sheets. Research confirms that TPU materials maintain >70% cell viability (meeting ISO biocompatibility standards) and are BPA-free, offering superior safety compared to some PETG-based alternatives.
✅ The Result:
Treatment has progressed favorably so far, and I continue to monitor closely for any complications. Clear aligners provided superior oral hygiene access compared to fixed appliances, reinforcing their value in this medically complex population.

With thoughtful protocol modifications, multidisciplinary awareness, and evidence-based material selection, we can safely expand orthodontic care to high-risk patients who might otherwise be deferred.
