Orthodontic Clear Aligners in Medically Complex Patients: A Breif Case Report on Renal Dialysis

Orthodontic Clear Aligners in Medically Complex Patients: A Breif Case Report on Renal Dialysis Managing orthodontic patients with renal failure on hemodialysis requires a careful balance of clinical precision and patient safety. These cases demand modifications at every level—from material selection to treatment mechanics. I recently managed a case using Maxflex™ trilayer clear aligners, and I'd like to share the key protocols that guided a safe approach. The Medical Problems: Altered bone metabolism Increased bleeding risk (due to heparin during dialysis) Higher susceptibility to plaque accumulation and infection Potential toxicity concerns from aligner sheet materials 📋 My Modified Protocol: Timing: Procedures were scheduled on non-dialysis days to minimize bleeding risk. While this was not always fully, due to the patient's fluctuating health status, I adapted accordingly. Plaque Control & Attachments: Based on clinical research and expert opinion, attachments were e...

The Concept of the Straight Wire Appliance in Orthodontics

There was no intention originally to produce a new appliance, but the need for one became evident.       The arising concept was to Pre-program suitable treatment into the appliance and therefore the SWA was developed. The built-in features of the SWA guide the teeth along direct vector lines, eliminating jiggling, round-tripping and other excessive movements. Andrews’s philosophy of treatment     A bracket type for each tooth type:       Since within any dentition, teeth differ from each other, therefore, each tooth would require its own bracket.    Measurements of the non-orthodo...

Non radiographic methods for skeletal age assessment

Image
if you are intersted in this article please follow for more readings https://myorthodonticsblogg.blogspot.com/        👈🏻 Introduction Every individual matures according to his or her own biological clock. Different authors had reported different methods in an attempt to determine the best indicator of maturity. These include height weight; chronological age sexual maturation , Frontal sinus, biological age or physiological age; Hand-wrist maturity Cervical vertebrae dental eruption; dental calcification stages, and recently biomarkers. Requirement of an ideal maturity indicator Should be safe and non invasive Require minimum radiation Should be accurate Stage of maturity should be well defined Easily identifiable Cost effective Require minimum armanterium Simple to conduct Valid overtime and across age group 1-Biological age or physiological age The physiological age of a person is determined by the degree of maturation of the different tissue syste...
